Every newly admitted student must complete and submit the following forms before advisement or registering for classes

Please submit all required forms before your advisement appointment, and keep a copy of your immunizations for your records.

HEALTH HISTORY FORM

This is found in the Patient Portal under “Forms”. Enter the information carefully and only hit the “submit” button once.

IMMUNIZATION PACKET

This consists of THREE sections and can be downloaded HERE.

PATIENT AGREEMENT FORM

This is found in the Patient Portal.

SUBMIT INSURANCE INFORMATION

Log into the Patient Portal, then click on “Insurance Details” in the left navigation bar.

Immunization Packet
Section 1: The Certificate of Immunization

Please fill out or attach GRITS (Georgia Immunization Registry) or other certified immunization record. If you provide a GRITS record or other certified immunization record you DO NOT NEED a healthcare provider’s signature. If you do NOT have a certified immunization record, and need to fill out Page 1, any healthcare provider’s signature is allowed. This can be a registered nurse, nurse practitioner, physician assistant, physician, pharmacist, Department of Public Health representative, etc.

Section 2: Tuberculosis (TB) Screening Questionnaire

Everyone must fill this out and return with your immunization packet

Sections 3 and 4: Clinical Assessment by Healthcare Provider

You ONLY need to complete this section IF YOU ANSWERED YES to any of the questions on the TB Screening Questionnaire.
Patient Agreement Form
If you are a student and will be under age 18 on the first day of classes: Please download and print the Patient Agreement Form and have your parents sign to give permission for the University Health Center to provide treatment. You must upload the printed and signed form to the Patient Portal (select “Immunization Upload”) before you can be seen for an appointment.
Transfer Students
Please be aware that your transcripts will not contain your immunization records.  Please send a copy of your immunization records from either your university/college or from your physician.  These must be completed and returned before you can register for class.
Out Of State Student?
Use this form to access your state's immunization records.

Policy
It is the policy of the University Health Center to comply with the mandatory immunization program established by the Board of Regents of the University System of Georgia. These requirements include documented proof of immunity to measles, mumps, rubella, varicella (chicken pox), tetanus, and hepatitis B, and completed screening questionnaire for tuberculosis prior to registration at the University of Georgia. All entering students must meet these requirements, and no clearances are given to allow registration without meeting these requirements. For additional information or assistance, contact UHC Registration and Health Information department at 706-542-8617.

The following Certificate of Immunization and TB Screening Form must be printed, completed, signed by your doctor, and returned to the health center before you can register for classes. Please keep a copy for your records: Immunizations Packet - Certificate of Immunization Form and Tuberculosis (TB) Screening Form (These forms are in PDF format. You must have Adobe Reader in order to view and print. Download Adobe Reader.)

Immunization Requirements
Listed below are the immunization requirements for students entering the University of Georgia. Please review the chart below for additional recommended vaccines. All required vaccines are available at the University Health Center. Please check in at Patient Registration.

NOTE: If TB testing is recommended based on the Tuberculosis Risk Assessment Form, failure to comply with testing recommendations will result in the inability to register for second semester.

Required Immunizations
MMR (Measles, Mumps, Rubella)Applies to students born in 1957 or later. Two doses or titers with results attached are required. First dose of MMR must be given after the first birthday with the second dose at least 28 days later.
Varicella (Chicken Pox)Applies to U.S. born students born in 1980 or later, and all foreign born students regardless of year born. Two doses are required, or titer with results attached. The second shot must be given at least 4 weeks apart.
Tetanus/Diptheria (Tdap)All students must have one dose of Tdap and a Td booster if the Tdap is more than 10 years prior.
Hepatitis BAll students 18 years old or younger at the time of matriculation. 3 doses are required, with the second dose given 1-2 months after the first dose, and the third dose given 4-6 months after the first dose.
TB (Tuberculosis screening)All students must complete the “Tuberculosis (TB) Screening Questionnaire (Required), page 2 of the immunization form, and return this to the University Health Center. All students with a “Yes” answer to any question must also complete page 3, “Tuberculosis (TB) Risk Assessment,” and return this to the University Health Center.
Recommended Immunizations
COVID-19The University of Georgia recommends that all faculty, staff, and students receive a COVID-19 vaccine. UHC administers Pfizer, Moderna, and Janssen (Johnson & Johnson) to students, faculty, and staff.
Hepatitis A2 doses are recommended for students who will be traveling abroad and with specific medical conditions.
HPV (Human Papillomavirus)
InfluenzaFlu shots are recommended each year at start of influenza season (October – March) and are strongly recommended for students with chronic medical conditions as well as those living in residence halls or other group living situations, or if a member of an athletic team.
Meningitis (Meningococcal disease)Freshmen or other new students who will reside in university housing or in sorority or fraternity houses are required to sign a document indicating that they have received the Meningitis ACYW-135 vaccine or have reviewed information about the disease. This documentation is included in the UGA housing contract. Meningitis ACYW-135 is strongly encouraged for all students under age 22, with a second booster does prior to admittance if it has been more than 5 years since the initial dose. Meningitis B is a different vaccine. It is a 2 or 3 dose vaccine that should be considered for unvaccinated students younger than 23 years.
